Job Summary and Qualifications
The RN Case Manager will be responsible for coordinating comprehensive patient-centered care, managing resource utilization efficiently, and ensuring safe discharges through teamwork with a multidisciplinary approach.
Key Responsibilities:
- Develop and document a personalized case management plan addressing clinical, psychosocial, and discharge needs.
- Lead interdisciplinary teams in assessing care levels and ensuring appropriate resource utilization.
- Evaluate medical necessity for admissions, addressing status issues as they arise.
- Communicate effectively with patients, families, and healthcare providers to facilitate care transitions.
- Document care coordination activities to maintain clear communication across the healthcare team.
- Involve patients and families in goal-setting to ensure alignment with discharged care needs.
- Assist patients with post-discharge services to support their ongoing health needs.
Required Qualifications:
- Active Florida RN license or multistate compact license with relocation completion within 60 days.
- BLS certification from an AHA-approved provider required within 30 days of employment.
- Associate's Degree or equivalent in Nursing; Bachelor's Degree preferred.
- Minimum of three years of nursing experience in an acute care setting.
- Experience in Case Management is preferred but not required.
